The more you tell us on what a client needs to have with them to apply, the more we can help them understand what to expect when applying. This benefits not only the client but you, the service provider, also. We want to not only help the client but cut down the added request for information from the client needed to qualify them for the program. When you provide us with what you require not only in guidelines on your program's eligibility but also what documents are needed, it not only saves the client time but saves your staff some time too! To maintain this data, we need your participation in representing your information correctly and completely. To see if your Agency has listing with us go to our "Service Finder" page on the tool bar at the top of this page and type your agency's name in. Any programs and services offered by your agency should come up. If your Agency has services that we have listed or should have listed you can now "update" online with our new fillable Online Survey Monkey !!! This will make sure your clients, have the most current, up-to-date information when we refer them to your Agency. And not only is filling out the form eaiser but so is getting it back to us!
